Recovery often continues after you leave the hospital. Our skilled staff provides the care you need to help you improve at home.
Virtua Home Health Care provides high-quality, at-home care after a hospital or rehabilitation center stay, or to help you manage a chronic illness. Our staff focuses on your recovery, helping you get stronger, readjust to daily life, and prevent repeat hospitalizations and unnecessary emergency room visits.
Our Approach to Home Health Care
After receiving an order from your doctor, a nurse will call you within 48 hours.
Our compassionate staff continues the high level of care you received in the hospital or other medical facility. We monitor your progress and carry out any changes to your treatment as directed by your doctor.
Your registered nurses, therapists, social workers, and dietitians work closely with your family and physician to ensure you receive the care you need.
Virtua Home Health Care staff care for people who:
- Have recently been discharged from the hospital
- Are recovering from an injury or illness
- Are managing a chronic condition or disability
Our Home Health Care Services
We serve people of all ages with the following services:
- Skilled nursing
Registered nurses provide a wide range of care, as well as easy-to-follow education to help you care for yourself. Guided by a comprehensive plan of care, our nurses monitor your condition, help you manage your medications, and perform ordered treatments and procedures. We provide specialized care for the treatment of heart failure, stroke, wounds, and incontinence. - Physical, occupational, and speech therapy
Therapists create an individualized plan of care to improve your overall function and mobility. - Nutrition support
Registered dietitians will create a meal plan that aids your recovery and wellness. - Social work
Social workers help you cope with issues related to illness, the recovery process, and body image following amputation, mastectomy, or other major changes. They make sure you understand your recovery process and help find community resources for continued rehabilitation and financial coverage, if needed.
Visits generally occur once or twice a week for six weeks.
Virtua Home Health Care accepts Medicare, Medicaid, and most private health insurance plans.
